Patient Registration

We are currently accepting new patients who live within our practice area which incorporates most of Herne Hill up to Brixton and across to Camberwell and East Dulwich.

Catchment Area

When you register with a surgery, the surgery is obliged to be able to visit you at your home address. It is for this reason that Brockwell Park Surgery is not able to accept registrations from outside our catchment area until this has been sorted by NHS England. The surgery appreciates that many patients never request a home visit but we have a legal obligation to provide this to your registered address.

New Patient Registration

To register with the practice, please complete our New Patient Registration form.

When you submit the patient registration form, you need to provide us with recent proof of address (a recent utility bill, council tax form, bank statement, tenancy agreement). You will need to bring this into the practice and sign the New Patient Registration form.

The registration process takes 2 working days, so you will not be able to see the doctor or nurse on the day you register.

If you were previously registered with the NHS, we would strongly advise you to write your NHS number on the registration form. If you do not know your NHS number, please contact your previous GP and they should be able to provide you with this.

You do not need to inform your old practice that you are leaving, but it may be polite to do this.

If you move out of our practice area, you will need to register with a new GP in your area. In most cases you will have 6 weeks to find a new Practice in your new area before you are deducted from us.

Also, please try to remember to keep all your contact information with us up-to-date so that we can contact you when necessary. You can keep your patient details up to date by completing our Change of Details form.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.

You cannot register as a temporary patient at a practice in the town or area where you are already registered.
