Please note that your COVID-19 vaccination status can be accessed automatically through the NHS App without the need for Brockwell Park Surgery to grant you online access to your GP record.

If you do not have a smartphone and need proof of your COVID-19 vaccination status, you can do this by creating an online NHS login account at www.help.login.nhs.uk/setupnhslogin
Alternatively, you can call the NHS helpline on 119 and ask for a letter to be posted to you.
In line with national guidance, GPs are unable to provide letters about your Covid-19 vaccination status.
Patients aged 13 and above can access the NHS App. Patients aged 13-15 will need their access authorised by a GP first – please Register for Online Services and follow the instructions on the form, so that this can be arranged.
Children aged 5 and over will be able to access the digital NHS COVID Pass via NHS: Covid Pass.
