Repeat Prescription Request

If you have access to online services (e.g. via the NHS App), please request your repeat medication in this way, by clicking on the ‘prescriptions’ icon at the bottom of the screen after login. This is the quickest way to get your prescription authorised. You will also be able to see when your prescription has been approved and is ready for collection, via the ‘view your orders’ tab.

If you do not have online access, please download the NHS App and follow the instructions on the app to prove who you are using photo ID.

If you are not able to access the NHS App, you can use this form to request any repeat prescriptions from the Practice.

Please allow 2 working days before collecting your prescription.

We will not contact you to confirm that your prescription has been issued. If there is a problem with your request, we will contact you.

If you are requesting a repeat prescription for the contraceptive pill and it has been a year since you last submitted a pill review form, please complete and submit this agai.

Prescriptions for over-the-counter medicines

  • You can buy over-the-counter medicines in pharmacies, supermarkets and some high street shops.
  • GPs, nurses and pharmacists based at the surgery will not generally give you a prescription for over-the-counter medicines.
  • Your local pharmacy offers help and clinical advice to manage minor health concerns. If your symptoms suggest it’s more serious, they will help you get the care you need.
